REQUIRED REGISTRATIONS

  • Unemployment Insurance: Arizona Department of Economic Security (DES)

  • Income Tax Withholding: Arizona Department of Revenue (DOR)

REQUIRED INFORMATION

  • SIT ID # (format: XX-XXXXXXX)

  • SIT Payment Frequency

  • SUI ID # (format: XXXXXXX-X) 

  • Current year SUI rate

  • Form 821-PSC

    • Arizona businesses that use a payroll service provider are required to file a Form 821-PSC . The form authorizes Namely to file returns or make payments on your behalf. Once completed, it is kept on file at the Arizona Department of Revenue. 

    • Namely will file it with the Arizona Department of Revenue on your behalf. Please see instructions  and FAQs for your reference. Please send the completed form and any questions to us by submitting a case in the Help Community.

     

REGISTERING FOR STATE UNEMPLOYMENT AND INCOME TAX

Go to the Arizona TPT License Registration website, and follow the prompts to complete user registration. Be sure to keep all pertinent information for your records. DES and DOR process your application separately, and each agency will notify you once the processing is complete.


TIP:

Future-date registration is possible for this state's unemployment insurance and income tax withholding!

Once registered, you will receive both an Unemployment Account Number (UI)Unemployment Rate (%) and Withholding Tax Account Number (WH) which you will need to enter into Namely Payroll under Company > Tax (UI & WH tax). Your Arizona Account Number will be 8 digits. If you have run payroll in Arizona in the past, you can find your account number on notices received from the Arizona Department of Economic Security or call the number below.

POWER OF ATTORNEY FORMS

Please note, Namely does not require a Power of Attorney for this state at this time. It is not required to file or remit payments on your behalf. If one is needed to resolve a tax matter, the tax notice team will request one from you at that time.

If a Power of Attorney is requested from you, you should email completed copies of the forms 
namelyPOA@Namely.com with the CID in the subject line for processing. We will update our system and maintain a copy of each form for record-keeping purposes

STATE INCOME WITHHOLDING FORMS

Namely uploads Federal and State W-4 documents to your HRIS site for use during onboarding, or if you need to send W-4s to employees who need to make withholding adjustments. 

TIP:

The most up to date W-4 documents can be found in the Namely section of the Document dropdown menu in Onboarding and Manage eSignature


Form Name: A-4

Accepts Federal W-4: No

Comments: Employees can indicate a withholding rate by filing Form A-4, Employee's Arizona Withholding Election, within five days of employment. Lacking this form, the employer should withhold at 2.7%. A new Form A-4 must be filed if the employee changes to another withholding rate.
